PROGRAM DESIGN: Roads to Success (RTS) facilitates a variety of youth programs including after-school, summer camps, retreats, conferences and summer employment opportunities reaching thousands of young people throughout NYC each year. RTS emphasizes the youth development practice of the Circle of Courage in all our programs; creating an environment of Belonging, where young people can build their Independence, find and develop their Mastery skill, and practice Generosity in their communities. At the core of our programs are staff training strategies that enable our young professionals to develop extraordinarily meaningful relationships with the young people in their care who we call Our Future Leaders because that is what they all are.
PRIMARY FUNCTION:
The Student Case Manager plays a crucial role in providing tailored support to high school juniors, seniors, and first-year college students who require assistance with basic needs. This position requires expertise in case management, social work, and connecting students with community resources. Working closely with students, faculty, staff, and community partners, the Student Case Manager ensures individual needs are addressed to promote student success.
ORGANIZATIONAL ROLE:
Supervisor: Program Director
Hours: Full-time (35 Hours/Week)
Work Schedule: Monday – Friday, 8:00am – 4:00pm (Some weekends)
Compensation: $25/Hour Non-Exempt

ESSENTIAL FUNCTIONS:
- Develop and maintain a comprehensive database of referral resources for social service agencies within the local community.
- Conduct initial intake meetings with program participants to assess their needs and provide appropriate referrals to city services and resources.
- Collaborate with academic leaders and program coordinators to address any behavioral issues that may impact participants' engagement.
- Coordinate support services, including housing, mental health, financial assistance, and educational resources, tailored to each participant's needs.
- Assist participants in accessing city and community services, ensuring compliance with program requirements and recommendations.
- Provide crisis intervention for participants in immediate need of support, such as shelter, food, financial assistance, transportation, or legal aid.
- Develop personalized case plans for participants based on their individual circumstances, preferences, and goals.
- Collaborate with program staff and city agencies to implement and adjust case plans as necessary to support participant progress.
- Conduct regular check-ins and follow-up meetings with participants to monitor progress and address any emerging needs or challenges.
- Document case information and updates securely and confidentially in program databases and records.
- Work collaboratively with other program staff and city agencies to ensure participant safety, well-being, and program compliance.
- Develop and deliver virtual resources, training materials, and outreach programs to enhance participant engagement and access to services.
- Participate in ongoing meetings and training sessions focused on program improvement and participant support.
QUALIFICATIONS AND EXPERIENCE:
- Bachelor's degree in social work or a related field.
- Experience working with city systems such as public assistance, child welfare, education, and housing.
- At least three (3) years of successful experience in providing case management, counseling, or related services.
- Understanding of participant needs and challenges within city systems, particularly in the context of education and work experience programs.
- Strong analytical, organizational, and interpersonal skills.
- Ability to exercise sound judgment, discretion, and empathy in working with program participants.
WORKING CONDITIONS
- Work takes place in a NYC Public High School which may require climbing stairs.
- Travel via public transportation or car may be required throughout NYC.
- Ability to travel between the program site and other locations for meetings and professional development.
- Flexibility to work outside regular office hours for special programs and events.
- Typical job hours are 8 am - 4 pm, Monday - Friday, and events will take place during occasional nights and weekends.
- Comfortable working in dynamic and fast-paced environments.
